Spine Week 1 Case 4

Normal alignment and segmentation is maintained. There is prominent disc degeneration at L4/L5 and L5/S1. Generalised L4/L5 disc bulge with a focal central and paracentral disc herniation just abutting the thecal sac. There is disc extrusion with a sequestered component compressing the right L5 nerve root just before it exits, at the most inferior aspect of the lateral recess. The nerve here is swollen with rather indistinct peripheral margins. No exit foraminal narrowing is seen.
At L5/S there is a generalised disc bulge with an extruded component directed cranially. This abuts the ventral thecal sac but no overt S1 nerve root compression is seen. The exiting left L5 nerve root just touches the lateral component of the disc without overt compression.
