There is mild straightening of the lumbar lordosis, otherwise sagittal alignment is maintained. Transitional lumbosacral junction noted with a right-sided pseudarthrosis between the L5 transverse process and sacrum (castellvi 2a). Normal segmentation otherwise.
L4-L5 posterior fusion shows satisfactory positioning and alignment of the bilateral pedicular screws. No screw fracture or osteolysis is seen.
There is a defect in the superior endplate of L3 centrally with acute oedema involving the intervertebral disc and vertebral body. The oedema is confined to only one side of the endplate and the location is rather typical for an acute schmorls node.
