• There is marked loss of bone at the level of the L3 vertebra where there is left lateral subluxation of L2 on L4 and an anterior cage extending obliquely from L2 to L4 but clearly showing non-union with adjacent endplates.
• Some bone graft has been placed on the left side of the cage and although this may be united to the inferior endplate of L2, it does not show any definite continuity with L4. Pedicle screws have been placed between L1 and S1. The right L2 pedicle screw is severely medially angulated and traverses the anterior aspect of the lateral recess and may interfere with the traversing right L2 nerve root.
• The right and left L4 pedicle screws are passing through fragmented bone but do not appear to be involving the canal significantly. The L5 and S1 pedicle screws are well positioned. The T12-L1, L1-L2, L4-L5 and L5-S1 disks appear fairly well preserved. It is very difficult to comment on the status of the canal at the fracture level and this would require an MRI study.
• There is extensive calcification seen in relation to the dura below the L4 level, the features being those of arachnoiditis ossificans.
