• Instrumented fusion has been performed in the lumbar spine and sacrum, with bilateral pedicle screws in situ from L3-S1, left lateral rod and screw fixation distally on the left to L3, and continuation of the posterior rods cranially, outside the field of view of this study.
• There is a fracture of the right-sided posterior rod at L3-L4 and L5-S1, with discontinuity of the left-sided rod at L3-L4. Interbody cage is present at L4-L5. The screws appear intact, with satisfactory positioning.
• There is a pseudo-arthrosis at L3-L4. Anterior and posterior fusion is present at all other levels within the field of view. Left lateral vertebral body osteophyte formation is resulting in narrowing of the left L5 neuroforamen. Osseous spinal canal and foraminal dimensions otherwise appear capacious.
• Summary: Fractures of the bilateral posterior rods at L3-L4, with a further fracture on the right at L5-S1. Pseudarthrosis at L3-L4.
