Spine Week 11 Case 4

• There is a low T1 signal fracture line traversing through the anterior superior endplate of L1 with associated osteoporotic wedging (roughly 25% height loss). The appearances are suggestive of an osteoporotic endplate fracture, however there is gross surrounding oedema. This involves the entire body as well as the anterior pedicles bilaterally which is slightly atypical. Reassuring however, some intrinsic fatty signal is preserved within the vertebral body itself and no destructive/expansile lesion is seen. No posterior cortical bulge or retropulsion is identified.
• Conclusion: Osteoporotic superior endplate L1 fracture. The amount of surrounding oedema is slightly atypical however. I would recommend a repeat 3 month short interval MRI to hopefully highlight that the oedema is resolving. There is no suggestion of a bone sarcoma.
