History: 40 years old. Progressive and recurrent episodes of limb weakness.
Findings: At C2 there is a well-defined expansile intramedullary mass which has heterogeneous internal signal and a surrounding haemosiderin rim. It demonstrates internal regions of T1 hyperintensity blooming artefact on gradient-echo-based sequences in keeping with previous haemorrhage.
There is only a trace of vasogenic oedema in the adjacent cord.
Opinion: The appearances are consistent with a high cervical cavernous venous malformation (“cavernoma”).
