• A cystic abnormality is seen at the L2 level, assuming the transitional lumbar vertebra is taken to represent L5. The lesion is rather lobulated and involves the posterior elements. No peripheral rim of sclerosis, perilesional oedema or other concerning features are seen.
• There is mild degeneration with a bulge/central protrusion at L4/L5 but the lumbar spine otherwise appears unremarkable with no evidence of lumbar nerve root compression. Satisfactory appearances of the distal thoracic cord and conus. There is prominent right-sided osteitis condensans, and a bulky fibroid uterus is noted.
• Conclusion: unusual cystic lesion as described above-differentials include secondary ABC, cystic fibrous dysplasia.
