Spine Week 14 Case 1

• There is an atypical scoliosis with a swooping right thoracic curve, centred over the lower 3 lumbar intervertebral discs which show marked disc degeneration and Modic endplate changes. The right-sided truncal shift as seen on plain radiographs is mainly centred over the L3/L4 disc. Compensatory left upper thoracic curve. Otherwise no transitional lumbosacral junction is seen, with normal vertebral body segmentation throughout. No features of spinal dysraphism or neuro axial abnormality. There are 12 pairs of ribs. Normal appearances of the craniocervical junction.
• No chiari malformation or syrinx is seen. The conus is at T12/L1, with normal signal of the visualised cord otherwise. There is no focal or diffuse marrow signal abnormality identified. The intra-abdominal viscera (left kidney not visualised) and paraspinal soft tissues are normal. Previous right L4 and L5 hemilaminectomy.
