Spine Week 15 Case 7

• There is an incidental schmorl node present in L3/L4 with an associated degenerate disc.
• There is a subtle area of low T1 signal within the central and posterior aspect of the L3 vertebral body which appears to extend into the left foramen as well.
• On T2 and stir sequences there is a convincing bulge at the posterior cortex with slight hyperintensity. There is further prominent oedema within the adjacent psoas muscle.
• The post contrast study (6.5) shows interval collapse of the L3 inferior endplate with size increase of both the vertebral body and extra osseous component which now extends further into the right psoas muscle. A CT biopsy of the latter is recommended, with the main differentials being metastases or primary bone tumour. (Final diagnosis-ewing sarcoma)
