Spine Week 2 Case 6

Rudimentary disc at S1-2. Lumbar spinal alignment is satisfactory and no obvious complication is seen in relation to the L5/S1 instrumented fusion. The remaining lumbar disc show a mild degree of degenerative change with no significant bulge or protrusion.
• but the combination of hypertrophic facet OA and ligamentous thickening is resulting in moderate to severe central canal stenosis at L3/L4 but no definite cauda equina compression in the supine position.
• Hypertrophic facet OA is also noted at the L4/L5 level.
Conclusions: Possible occult L3/L4 central canal stenosis which could be investigated further with an Upright MRI scan if the patient is a candidate for further decompressive surgery.
