• There is a transitional lumbosacral vertebra. For reporting purposes I will call the last mobile level L5-S1. The bilateral transverse processes of L5 articulates with the sacrum. There is evidence of bone marrow oedema on either side of the pseudoarticulation on the right-starting to fuse on left in keeping with castelvi-2b transitional level. There is associated bilateral L4-L5 facet joint oedema.
• Endplate changes characterised by mild bone marrow oedema and a small Schmorl’s node is demonstrated at T12-L1. Early disc degenerative change characterised by loss of the normal hydration is noted. Minor circumferential disc bulge is also present at this level. No central canal compression. No exits foramina compromise.
• Otherwise, the visualised discs are unremarkable without convincing evidence of degenerative change, disc bulge or protrusion.
• Conclusion: Transitional lumbosacral vertebra with bilateral pseudoarthoses with acute oedema on the right side. Bilateral l4/l5 facet oedema in keeping with underlying pars defects
