Spine Week 3 Case 7

• The physiologic cervical lordosis is relatively preserved. Multilevel spondylotic change noted. Normal signal in the visualised cord.
• The atlantodental distance is approximately 6 mm in neutral position, 8 mm in flexion and 5 mm in extension. The approximate increase in distance between flexion and extension is 3 mm. Findings are suggestive of a degree of instability
• Multilevel disc degenerative change noted which is more evident at C3-C4 with evidence of a small circumferential disc bulge that in combination with ligamentum flavum hypertrophy causes minor narrowing of the central canal. Subtle cord signal change at c3/c4-myleopathy. Minor narrowing of the bilateral extra foramina at this level is also noted (R>L) without evidence of nerve root impingement.
• Impression: 1. Widening of the atlantodental distance in neutral position. The 3 mm distance between flexion in extension. Minimal spinal canal narrowing at C3-C4, worse on extension.
