Spine Week 4 Case 5

• At L2/L3 there is evidence of endplate sclerosis and erosive changes with disruption of the subchondral bone plate and associated intense bone marrow oedema. These findings correspond to the significant endplate erosive changes and bone sclerosis demonstrated on the radiographs of the 2/7/2021. There is loss of disc height and hydration without significant disc bulge. No fluid signal within the disc is noted.
• Degenerative change is demonstrated in the reminder of the visualised spine without evidence of central canal compression or neural foramen compromise.
• Impression: L2/L3 endplate changes, as described. The preferred diagnosis would be psoriatic spondylitis. Clinical correlation advised.
