• There is a lesion within the L4 vertebral body showing T1 hypointense, T2/STIR hypointensity with well defined sclerotic thick margins. There is intralesional hyperintense signal with normal surrounding marrow fat. Intact adjacent cortices and endplates and no overt bony expansion is seen. The main differential diagnosis here is fibrous dysplasia.
