Spine Week 7 Case 1

• There is a solitary marrow-replacing expansile T1 hypointense and T2 hyperintense lesion involving the L2 vertebral body with associated biconcave pathological collapse. There is erosion of the superior and inferior endplates and posterior cortex as well as the extraosseous extension of the lesion into the ventral epidural space. There is mild left lateral recess stenosis. No spinal canal or foraminal stenosis.
• MRI and CT shows low signal intensity thick cortical struts (better seen on ct) consistent with a mini brain appearance characteristic of plasmacytoma
